This form has 5 sections. It is designed to be completed by memory in 3-minutes or less.
 
Space is limited and participation in this program is at the sole discretion of the Program Facilitators. After submitting your application, you will be notified shortly (via email) regarding application status.

Section 1 ~ The Basics
 
Program Terms
 
Program participants are expected to attend small group coaching meetings, once per month (virtual attendance is offered).
 
Membership may be terminated due to lack of participation and/or adherence to program guidelines.
 
Additional group gathering opportunities are optional and event fees for WVS Members are typically covered (unless otherwise noted).

Statement of Commitment
 
As an individual who has experience serving in the US military, I commit myself to furthering the values of such as they are reflected in the ideals and principles of this program. To the best of my ability, I will:
  • Refrain from harming myself or others.

  • Respect and support others in their task of self care.

  • Respect members and support staff, and support them in maintaining the Program Guidelines.

  • Serve as a positive advocate for female military in [the] community and society.

  • Engage in an ongoing process of self-reflection, realizing that personal characteristics, biases, and beliefs have an impact on others.

     

  • Be coachable, open to new ideas, and willing to learn from the suggestions of others.

  • Continue to learn, grow, and contribute as a member.

  • Honor the ideals and principles of this program.

Note: This Statement of Commitment is not part of the Code but is a personal acknowledgment of the individual’s willingness to embrace the distinctive values and moral obligations of this program. It is a recognition of the moral obligations that lead an individual to become part of a cohesive and healing community.

 

In addition, I commit to full participation in this program and agree to attend the required meetings, in accordance with program guidance. I will stay in regular contact with my Coach or risk being removed from the program.

Communication
 
This program teaches forward-thinking content in each group coaching session. Then, we reinforce this lesson throughout the month through text messaging.  We only send event reminders and positive, engaging messages to your mobile device (with your permission, of course!).

 

As part of our program, we will send you regular text messages through our partners Others Over Self®  and Mission Ambition, LLC, to help recognize, teach, communicate, listen to, and inspire you.

  3. The security procedures in place to protect the misuse of your information.
  4. How you can correct any inaccuracies in the information. 
Information, Collection, Use and Sharing
We are the sole owners of the information collected on this form.  We only have access to/collect information that you voluntarily give us via email or other direct contact from you or from your Veteran Navigator in the Walking With Warriors program.  We will not sell or rent this information to anyone.
 
We will use your information to respond to you, regarding the reason you contacted us.  We will not share your information with any third party outside of our organization, other than as necessary to fulfill your request, e.g. to ship an order.
